Shoulder injuries are extremely common and often lead to chronic pain and limitations in range of motion. The shoulder is a complex joint with many moving parts including the rotator cuff tendons, surrounding ligaments and the labrum. What Causes Shoulder Pain?
Sometimes the rotator cuff tendons can become overstressed and inflamed, which can lead to a painful tendonitis or commonly can result in rotator cuff tears. The supporting ligaments can also become overstretched leading to decreased joint stability. This can result in a dull or stabbing pain in the shoulder itself or can radiate down the scapula or down the arm. If not properly treated the instability can increase the rate of degeneration leading to shoulder arthritis. Other times the shoulder pain is due to something else entirely like in the case of shoulder pain from fibromyalgia.
Symptoms and Diagnosis
People most often notice their shoulder pain when trying to sleep at night or with overhead activities. The pain is often accompanied by stiffness or weakness. Unfortunately, many patients will go on to develop neck pain and or upper back pain due to the body compensating for the loss of motion in the shoulder.
In order to accurately evaluate shoulder pain a through history and physical exam is of utmost importance. Many of our patients have been told that they need a shoulder replacement based off imaging alone, which can result in unnecessary surgeries. We do use advanced imaging such as ultrasound and MRI, but the physical exam is vital for making a proper diagnosis. Areas of pain need to be thoroughly evaluated and other factors such as muscle imbalance, ligament instability, motion restrictions, and areas of weakness need to be identified and corrected.
